Diabetes Projects Trust - DCSS Audit
The DCSS Audit is a special service provided free to General Practitioners in South and West Auckland, funded by the District Health Board.
The purpose of the Audit is to help GPs improve the care they provide to their patients with diabetes and gather non-identifiable statistics which may be used for service development by relevant organisations.
Services it provides include:-

* Maintaining a regional database.
* Developing and updating a comprehensive register of individuals with diabetes in a practice.
* Summarising patient demographics.
* Feedback on diabetes care targets with regional comparison for Quality Indicator development/planning.
* Identifying key clinical issues and improvement opportunities.
* Providing patient specific evidence based on specialist advice from the DCSS Executive group.
* Assisting with system development.
* Providing information for measurement of project effectiveness pre and post.
* Providing participating GPs with MOPS credits from RNZGP.
* Is at no cost to the GP.
* Compliments the national 'Get Checked' programme and the Counties Manukau 'Chronic Care Management' programme.


MANAGING PRIVACY

* Data is only used for the purpose it is collected.
* The collection of data is carried out by qualified Nurses trained in audit procedure and data gathering and all DPT employees who have access to the information have privacy training and a confidentiality agreement.
* Access to information is by authorised persons only and collection, handling and storage is in line with current Privacy legislation/DPT policy.
* Computer stored data is in a stand alone system (not Internet accessible), backed up daily, virus protected and password protected.
* Identifiable information gathered and specific analysis done is for feedback to the originating practice only unless agreement has been made with another organisation (e.g. PHO).
* Non-identifiable data may be analysed to produce regional statistics. Identifiable Patient/GP information is NOT accessible to any other organisation or body without express permission.
* Patients may view their information or ask for it to be withdrawn by notifying their GP or contacting the Diabetes Projects Trust.
* A poster is provided to the GP/Practice to inform patients of the audit. This poster must be displayed prominently.
